Resources
C.H.A.T. Community Resource initiatives support organizations in coordinating programs and connecting residents to vital services, supports, and pathways to wellness. Through collaboration and intentional partnerships, we help strengthen access to care while reducing barriers that often prevent individuals and families from receiving the support they need.
